Forms Upon enrollment, your child will need all 4 Our Kids forms completed in a timely manner. All forms will be provided by the Director at the time of enrollment. For your child’s safety, we will periodically ask parents to update their child’s information. You want to know what you need to enroll your child. All the forms are right here. Download them, print them and complete them when you have the time. Then bring them with you to 4 Our Kids Learning Academy
